Focus Area: Financial Stability – Family Case management
Program Description: The mission of Catholic Charities is to provide services that impact family, neighborhood, and society so that people’s lives are filled with hope. Catholic Charities envisions that all families in the Coachella Valley experience stability, safety, health and well-being.
The Family Case Management Program connects families with resources and support to move families beyond a point of crisis and instability, to a place where they are stable and have the capacity to meet their needs.
The program has been serving residents of the Coachella Valley for over 30 years, with work that is a collaboration between caseworkers and the family members to assess the strengths and challenges of each family, develop goals and objectives, evaluate options and services available to help meet the needs of the family, and generally provides ongoing support.
Desert Blind & Handicapped
Focus Area: Financial Stability – Transportation Services
Program Description: Desert Blind & Handicapped strives to create a transportation network that complements public, paratransit systems to ensure that everyone with a disability has effective access to transportation. The organization strives to help people with disabilities remain active and engaged in the community, and to avoid premature placement in assisted-care living facilities.
The program provides transportation for residents who lack access to public transportation routes, to help them access medical services, food shopping, counseling, and care centers. Services provided cover the cities of Palm Springs, Rancho Mirage, Cathedral City, and Desert Hot Springs. They primarily serve low-income seniors and disabled residents, providing much needed access to doctor appointments, food/grocery shopping, physical and mental counseling, and care centers for those with specialized needs.
Desert Cancer Foundation
Focus Area: Financial Stability/Critical Needs – Patient Assistance/Navigator Program
Program Description: The Desert Cancer Foundation is dedicated to serving residents in the Coachella Valley, who need assistance in obtaining screening, diagnosis, and treatment of cancer. The vision is to make sure no local resident goes without treatment due to inability to pay.
The Patient Navigator program helps provide best avenues of financial assistance and resource support for qualifying individuals in the midst of cancer treatment/ care. This includes financial assistance with insurance premiums, prescription medications, copays/coinsurance, in and out patient services, and Medi-Cal SOC.
The program serves as a model in its role of reducing the long-term cost burden that cancer patients face and provides financial assistance and case management to residents who are underinsured, uninsured, or lack sufficient funds to pay for their medical care.
FIND Food Bank
Focus Area: Financial Stability – Gap-Based Food Assistance
Program Description: FIND Food Bank is dedicated to relieving hunger, and the problems associated with hunger, through awareness, education, and mobilizing resources and community involvement. FIND’s vision is to support a community that is free of hunger and food insecurity.
The Gap-Based Food Assistance program distributes foods through a Mobile Food Pantry Truck, to underserved communities that lack access to existing food distribution sites. The gap-based distributions FIND Food Bank currently supports are in neighborhoods, micro-communities, and larger communities with few to no resources that otherwise support regular food-assistance distributions and/or lack of transportation to distribution sites. The locations include the underserved communities/areas of Sky Valley, Salton City, Oasis, Indio, La Quinta, Palm Desert, Thousand Palms and North Palm Springs.
Focus Area: Financial Stability/Critical Needs – Emergency Food Services
Program Description: Galilee Center of the Desert’s mission is to fulfill the needs of the most underprivileged and disadvantaged by providing food, clothing, and other basic needs and affirm their dignity with love, compassion, and respect.
The organization’s vision is to help reduce the poverty levels of the migrant farm workers in the eastern Coachella Valley. The program provides emergency services to residents who lack access to resources. Critical services include weekly food distribution, rental and utility assistance, infant supplies, senior care services, and school supplies for children.
Jewish Family Service of the Desert
Focus Area: Financial Stability – Senior Care Management
Program Description: The mission of Jewish Family Service of the Desert (JFS) is to provide for social service needs of the Jewish and general community throughout the Coachella Valley, with a commitment to promote the well-being of persons of all ages, incomes, and lifestyles. JFS offers an array of counseling and social services to residents in need, regardless of religion, age, income, gender, national origin, ethnicity, or sexual orientation; with the vision to improve quality of life of the clients served and the community at large.
The Senior Care Management program provides services to seniors 60+ years throughout the Coachella Valley. The goal is to ensure safety, health, and independence. This includes geriatric care, transportation, emergency food assistance, and socialization programs to reduce isolation, particularly to the elderly that are low-income and/or homebound.
Martha’s Village & Kitchen
Focus Area: Financial Stability – Food Services Program
Program Description: The mission of Martha’s Village and Kitchen is to help neighbors in need break the cycle of homelessness and poverty by promoting sufficiency through an innovative continuum of care, multi-disciplinary programs and partnerships. The organization envisions a community where all people have the opportunity to live their full potential and everyone has access to food, housing, healthcare, education, and the means to maintain them.
The Food Services program directly addresses hunger in the community by providing nutritious, well-balanced meals to both residents and neighbors, to homeless and impoverished individuals and families. By offering meal services three times a day, the program provides thousands of nutritionally balanced meals to our community’s most vulnerable. When hunger is taken care of, individuals are better able to cope with and overcome the underlying challenges of their situation, and work towards self-sufficiency.
Martha’s Village & Kitchen
Focus Area: Financial Stability – Transitional Housing & Employment Services
Program Description: The mission of Martha’s Village & Kitchen is to help neighbors in need break the cycle of homelessness and poverty by promoting sufficiency through an innovative continuum of care, multi-disciplinary programs and partnerships. The organization envisions a community where all people have the opportunity to live their full potential, and everyone has access to food, housing, healthcare, education, and the means to maintain them.
The Transitional Housing & Employment program supports individuals and families experiencing homelessness who are working to regain self-sufficiency, income and permanent housing. By providing a safe and comfortable home, coupled with career preparation and job-placement assistance, staff members work to motivate residents to strive for self-sufficiency. The program serves as a vital safety net for homeless individuals and families, offering 120 transitional beds for homeless individuals, including 95 beds for families with children and 25 beds for single adults.
